What are dermal fillers?

Dermal fillers are soft, gel-like substances that are injected under the skin. They are biodegradable materials that soften and reduce wrinkles temporarily, when injected into the skin.

They can address a number of common concerns including smoothing of deep under-eye circles, lifting of cheekbones, volumization of the lips, smoothing of lip lines and nasolabial folds (the creases that run from the side of the nose to the corners of the mouth), and rejuvenation of the hands.

The present use of dermal fillers has travelled long way from the initial days with constant and continuous study and research. Fat was one of the first soft tissue fillers to be used and is still widely used today.

TYPES OF FACIAL (DERMAL) FILLERS

Hyaluronic acid 

HA is the natural component of skin’s connective tissue. This is the most commonly used filler for wrinkles today, by the dermatologists and maxillofacial surgeons. The results typically last about six months to one year. Most popular brands available are Restylane and Juvederm.

Calcium hydroxylapatite 

This is another filler which is used in contouring the jaw line, restoring volume in the cheeks, and treating deeper wrinkles and skin folds. The results can last up to one year when used for contouring and three years when used to fill wrinkles.

One of the popular brands is Radiesse.

Fat grafting 

Fat from the lower abdomen or other areas is procured through liposuction and then can be injected into your cheek, temple, lips or forehead through small incisions. The procedure is more invasive and achieving the desired results may require more than one session of injections. Overfilling the site is mandatory to compensate for the body’s reabsorption of the fat. 

Permanent soft tissue fillers

They are used to smoothen deep wrinkles around the mouth. Body cannot absorb this type of fillers, reinjection is not needed. Permanent soft tissue fillers generally isn’t recommended as a first-time facial filler treatment. Artefill is a popular brand.

Poly-L-lactic acid

They are used to restore facial volume lost due to aging or illness. Two or three injections are usually required. The effects may last up to two years. Sculptra is a known brand.

Before doing Dermal Fillers

  • Avoid excessive exposure to the sun
  • Avoid taking a sunbath and using sunbeds
  • Use the relevant sunscreen while going out
  • Take the usual medications prescribed by the doctor
  • Discontinue supplements such as Vitamin-E and Fish Oil.
  • Discontinue  blood-thinning medicines, with your physician’s consent

PROCEDURE

Injection of facial fillers are done as an outpatient procedure. Usually its performed under local / regional anesthesia. Fillers are supplied as preloaded syringes, along with needles in the same package. 

Fillers are injected into the specific areas at different depths of the skin, connective tissue and muscles, depending on the desired results. Different gauge special cannulas are also used to deliver fillers through a procedure called subcision.

Dermal fillers are safe and effective in the right hands?

Like any other surgical procedure, getting dermal fillers also have minimalistic complications. Where does this leave the savvy consumer interested in noninvasive treatments to reduce the signs of aging? 

So never forget, finding the experienced maxillofacial surgeon or dermatologist to perform your dermal filler procedure is of utmost importance!!  

Recovery:

Usually the patient will be able to resume normal routine activities next day after the treatment. Minor side effects  may be present after the procedure;

  • Minor pain
  • Slight uneasiness
  • Minor swelling and bruising
  • Irregular appearance of the treated area
  • Numbness and redness on the treated area

The side effects enlisted above are of temporary nature.  

Post-Treatment Care:

  • Avoid excessive sun exposure
  • Do not put pressure on the treatment area
  • Keep the treatment area clean and hygienic 

General guidelines about what you can expect:

  • Before getting the injections, you may need ice, an anesthetic applied to the skin, or a nerve block (an injection).
  • Your doctor will inject the filler into the area, often giving you several injections to produce the best results. 
  • You may feel a stinging or burning sensation as the filler is injected. 
  • Most filler treatments require one office visit.

You can return to your everyday activities after leaving the office.

To get natural-looking results, dermatologists recommend notto overdofillers. For example, patients who have very thin lips should not get dramatically fuller lips. You will look more natural with a conservative approach.

Fillers offer temporary results and require maintenance treatment. Temporary fillers may last anywhere between 6-18 months.

At the site of injection, one may have minor side effects like

  • Redness
  • Swelling
  • Tenderness
  • Bruising (occasionally)

 They are temporary and tend to clear within 7 to 14 days.

Few more serious side effects reported are

  • Allergic reaction.
  • Skin discoloration.
